To receive you AKC registration papers follow the 3 steps below.
1) Scan proof and Email ludwigsms@aol.com or Fax me 413-473-2423 the proof from your vet that your pup has been neutered/spayed. Submit the form below at the same time you E-mail/fax me the proof from your vet.
2) Put NEUTER/SPAY along with your LAST NAME and your PUPS BIRTH NAME in the subject line of the email.
3) Submit the form below on the same day.

What is the DATE you Emailed or faxed me your pups proof of Neuter/Spay? Please try to do so the same day you submit this form. I only mail out papers 3-4 times a year so expect to receive your papers in about 90 days.
